Terms of Use
Our Terms of Use outline the rules and conditions for using Spring Health's platform and services, including your responsibilities as a member, our responsibilities to you, and other legal terms governing our relationship.
Informed Consent
Before you begin care with Spring Health, you'll review and acknowledge an Informed Consent document. This explains what to expect from care, including confidentiality protections, the limits of confidentiality (such as situations where disclosure is legally required), and your rights as a member receiving care.
Informed Consent and Parental or Legal Guardian Authority for Treatment of a Minor
If your child or dependent will be receiving care through Spring Health, you'll review and acknowledge the Informed Consent and Parental or Legal Guardian Authority for Treatment of a Minor document. This covers the same information as the standard Informed Consent, along with confirmation of your legal authority to consent to care on the minor's behalf.
Release of Information (ROI)
A Release of Information (ROI) form authorizes Spring Health to discuss or share your health information with a specific person or entity — for example, sending records to another provider, your employer, or a disability insurance company. An ROI does not give someone the ability to manage your account or appointments.
Appointment of Representative (AoR)
An Appointment of Representative (AoR) form allows another person — such as a spouse, family member, or legal guardian — to act on your behalf and help manage your Spring Health services (scheduling, canceling, or rescheduling appointments, requesting a new provider, etc.). Because an appointed representative is treated as an extension of you, they can also receive your records without a separate ROI.
