Requesting a Letter from Your Provider

International Considerations: Processes may vary by location, country or region. For members located outside the U.S., please contact your Care Navigator.

At times, you may need a letter or written documentation from your Spring Health provider as part of your care or for personal, educational, or administrative purposes. The information below explains what types of letters may be available and how to request one.

Important Things to Know

Spring Health providers offer clinical care and treatment. While some providers may be comfortable diagnosing certain conditions through clinical sessions, Spring Health providers do not conduct formal psychological testing.

Whether a letter can be provided is ultimately at the provider’s clinical discretion and depends on the nature of your request and your course of care.

Types of Letters a Provider May Be Able to Support

At a provider’s discretion, letters may be available for certain purposes including, but not limited to:

  • Gender Affirming Care documentation
  • Premarital education certification of completion
  • Treatment summaries (High-level summaries of treatment focus and participation)

Eligibility may vary based on your provider, length of care, and clinical appropriateness.

Requests That Are Not Supported

Emotional Support Animal (ESA) Letters

In line with Spring Health policy and Informed Consent, Spring Health providers are not able to complete documentation for Emotional Support Animal (ESA) requests. This includes pet-related accommodation documentation for landlords, employers, housing authorities, or similar entities.

To ensure alignment with our policies and scope of care, this type of documentation is not supported through Spring Health or by Spring Health providers. We understand that emotional support animals can play an important role in many individuals’ lives, and we recommend seeking a provider or service outside of the Spring Health platform that specializes in ESA evaluations and related documentation.

Weapons Purchase Clearance Letters

In line with Spring Health policy and informed consent guidelines, Spring Health providers may not conduct nor attest to an evaluation to support a weapons purchase clearance.

How to Request a Letter from Your Provider

To request a letter:
