How to Get Started with a Mental Health Leave or Accommodation Request

Taking time to focus on your mental well-being or adjusting your work environment is a proactive step toward long-term health. Follow the steps below to begin your mental health-related leave or accommodation request.

Step 1: Initiate Your Request with Your Employer

Start by contacting your HR department or absence management administrator. They will initiate your claim for an absence or accommodation and share the documents you’ll need completed to support your request.

Note: if you are unable to open this link, your organization may not support Mental Health Leave & Accommodation support through Spring Health. For any questions, please contact the Care Team. 

Otherwise, follow the guided steps in the platform to:

  • Upload blank mental health leave or accommodation forms to support your request.
  • Review and sign a Release of Information (ROI).
  • Schedule an MHLA evaluation with a certified provider.

The platform will guide you through each step, and once you’ve submitted your request, you can track your case progress directly within the tracker.

Ongoing Management

Mental health leave and accommodation cases require ongoing coordination:

  • Schedule ongoing care and MHLA evaluations based on your recommended care plan.
  • Work with your care provider and employer on your return-to-work or accommodation plan.

Important Notes 

  • Questions about claim approval status, deadlines, or determinations should be directed to your employer or leave/accommodations vendor. Questions about care or Spring Health documentation should be directed to the Spring Health Care Team.
  • An evaluation for a mental health-related leave or accommodation does not guarantee approval. Your leave/accommodation vendor is responsible for the approval or denial of your claim/request.
  • All MHLA paperwork must be coordinated through the Spring Health Care Team. If your MHLA paperwork requires corrections or is missing information, please notify the Care Team promptly so we can facilitate any necessary updates with your provider.
