How to Submit an ROI or AoR Form Without Email Access

We understand that not having access to email can present challenges, but we are here to support you in ensuring your care is coordinated smoothly. If you need to authorize the sharing of your information or appoint someone to facilitate your Spring Health services, you can complete these steps entirely through our digital support center or via mail.

Understanding the difference between an AoR and an ROI

Form Type Purpose

Release of Information (ROI)

Allows Spring Health to disclose information and records (such as appointment notes, provider letters, or completed paperwork) with an authorized third party. This does not allow someone to manage your account (such as schedule and cancel appointments).

Authorization of Representative (AoR)

Allows another person to act on your behalf and facilitate your Spring Health services (cancel, schedule, and reschedule appointments, etc.).

  • Third Party Representative: A trusted family member, spouse, parent, or close companion.
  • Legal Guardian or Appointed Legal Representative: Someone with legal documentation to demonstrate guardianship or legal representation (such as a court order or custody paperwork).

If you are unsure which form you need, an AoR is required when someone will be coordinating care on your behalf, while an ROI is used strictly for information sharing.

Step 1: Complete Your Form

Key Instructions for All Forms:
  • Date Format: Please ensure all dates are formatted as mm/dd/yyyy.
  • Signatures:
    • Wet signatures (pen on paper) are always acceptable.
    • E-signatures are only accepted if they meet e-Sign Act Requirements (must include a date/time stamp, geolocation, and IP address). 
AoR-specific instructions
  • Complete questions 1–3 with your information.
  • In Section 1, select the checkbox that matches your request.
  • Complete either Section 2 or Section 3:
    • Section 3 must be completed by the Legal Guardian or Appointed Legal Representative, if applicable.
  • If you do not have an email address, indicate “N/A” where email is requested.
ROI-specific instructions
  • Clearly list the information you are authorizing Spring Health to share.
  • If you want all of your care eligible for disclosure:
    • Use the start date of your care and choose an end date far enough in the future (for example, one year out).
  • If you want copies of the disclosed information, add yourself as a recipient under “Disclose my information to.”

Step 2: Submit Your Documents

Once your form is signed, you can submit it using one of the following methods:

Method A: Digital Submission (No Email Required)
  1. Visit the Spring Health Support Center.
  2. Indicate "N/A" for the email address.
  3. Select Paperwork Requests, then choose Other.
  4. In the Description field, include:
    1. Your first and last name
    2. Your date of birth
    3. The phone number associated with your Spring Health account
  5. Upload the completed and signed form under the "Attachments" section and click submit.
Method B: Mailing Your Forms

If you are unable to submit your form digitally:

  1. Download and print the AoR or ROI form.
  2. Complete all required sections and sign the form.
  3. Mail your completed and signed documents to:
